New Patient Information

Before your first visit, you need to complete new patient information forms. You can either download and print the forms from this page or, when you schedule your first visit, you can ask our receptionist to mail them out to you. Please bring completed forms to your first visit. If you are not able to access the forms from the website and you don't have time to receive them in the mail, we request you come to your first office visit 15 minutes early in order to complete the forms. The forms are a valuable part of your medical records as they provide the physician with information regarding your current and past medical history, the reason you are coming to see the physician, your current address, telephone number, and insurance information.

At your first office visit, please bring the following:

• Current insurance card
• Any test results pertaining to the condition for which you are being seen
• If x-rays have been done, please bring a copy of the films. These may be obtained from the facility which performed your test (contact them regarding release requirements).
• Bring referral from you primary care physician. Click here for insurance information
• If your insurance has a copayment to see a specialist, payment is expected at the time of the visit.
