To schedule appointments, check on referrals, speak to the nurse regarding concerns of your condition, test results, medication refills, surgery questions, and billing questions, please call the numbers below.

HMO patients must contact their primary care physician (PCP) to obtain a referral for your office visit.

Our staff is comprised of well trained registered nurses, licensed practical nurses, medical assistants, medical secretaries, billing specialists, a registered diagnostic medical sonographer, and transcriptionist. Our goal is to ensure that you receive optimal medical care, an understanding of your condition and its treatment, and coordination of care with your primary care physician.

Our specialists in the billing department are able to assist you with insurance needs as it applies to your care rendered by Urology & Ultrasound Associates.
